Eligibility Criteria
You may qualify if you...
- Infant born alive with birth weight 1500 grams or less or gestational age 29 weeks 6 days or less
- Infant born at or admitted to a Vermont Oxford Network member hospital within 28 days of birth
- Infant has not gone home before admission to a member hospital
- Infant receives care regardless of hospital location within the member center
You will not qualify if you...
- Infant is stillborn
- Infant discharged home before admission to a member hospital
- Infant admitted after 28 days of life
