Eligibility Criteria

Eligible

You may qualify if you...

  • Child aged 0 to 12 years
  • Diagnosed with any type or stage of cancer
  • Currently receiving cancer-directed treatment
  • Parent is the decision maker for the child
  • Parent is a biological parent, step-parent, legal guardian, or grandparent with medical consent authority
  • Parent has a child meeting the above child criteria
  • Parent can communicate in English or Spanish
Not Eligible

You will not qualify if you...

  • Child is not receiving primary medical care from the cancer clinic (e.g., only second-opinion consultations)
  • Child has already been fully consulted by the palliative care team
  • Child has a prognosis of less than 2 months life expectancy
  • Parent has visual impairment worse than 20/200 corrected and cannot view the video
  • Parent's psychological state is not appropriate for Goals of Care discussions, as determined by the primary oncologist
  • Parent does not speak English or Spanish
