Eligibility Criteria
You may qualify if you...
- Age 60 years or older
- Resident of Qujiang District
- Diagnosed with chronic cardiovascular disease including hypertension, coronary artery disease, heart failure, atrial fibrillation, or others
- Able and willing to provide informed consent
You will not qualify if you...
- Severe cognitive impairment or inability to provide consent
- Life expectancy less than 1 year due to non-cardiovascular disease
- Refusal to participate
