Who Can Participate
Eligibility Criteria
You may qualify if you...
- A1C greater than 7 for ages 18 to 64, or greater than 8 for age 65 or older within 12 months before baseline
- A1C greater than 7 for ages 18 to 64, or greater than 8 for age 65 or older at baseline
- Medicaid recipient or dual Medicaid and Medicare recipient
- Age 18 years or older
- Diagnosed with Type 2 diabetes
- Currently using insulin therapy
- Own and have used an Android or iOS smartphone for at least 6 months
- Able to read text messages on a smartphone
- Proficient in using a smartphone beyond emailing, texting, and calling, as self-reported and confirmed by phone
- Proficient in English

- Pregnancy
- Cognitive impairment as identified by the Six-Item Screener
