'My Voice' is a website intervention developed to enable advance care planning (ACP) conversations for patients with kidney disease on dialysis, educate patients on their illness, help identify their goals and preferences for EOL care, and coaches them on how to speak with their HCPs and loved ones. This study aims to evaluate if 'My Voice' intervention improves quality of ACP conversations between patients and providers, and patients and family carers; improves patient's knowledge about kidney failure, engagement in ACP and preparedness for future end of life care compared to usual practices for ACP in dialysis centres.

Eligibility Criteria

Eligible

You may qualify if you...

  • Singapore citizen or permanent resident aged 21 years or older
  • Diagnosed with chronic kidney disease stage 5 and receiving dialysis
  • Eligible for serious illness conversations by healthcare providers based on clinical deterioration, dialysis intolerance, frequent hospitalizations, palliative or conservative care, or poor functional status
  • Normal cognitive function with Mini Mental Scale Examination score of 24 or higher
  • Able to speak and read English, Mandarin, or Malay
  • Agreeable to be audio-recorded
  • For caregivers: aged 21 years or older and primary family caregivers providing care or making care decisions without financial compensation
  • Caregivers must be able to speak and read English, Mandarin, or Malay
  • Caregivers must have a phone or electronic device with internet access
  • Caregivers agreeable to be audio-recorded

You will not qualify if you...

  • Patients with cognitive impairment or serious mental illness
  • Patients who have had prior serious illness conversations
  • Patients not easily contactable via phone
  • Caregivers who are foreign domestic workers or maids
  • Caregivers not easily contactable via phone
