The study includes ongoing monitoring of participants' experience and feedback, with a total participation period aligned to these timeframes.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adults aged 18 years or older
- Fluent in English
- Able to provide informed consent
- Able to complete questionnaires independently or with assistance
- Diagnosed with serious illness defined as a health condition carrying high risk of mortality
You will not qualify if you...
- Untreated schizophrenia or bipolar disorder
- Recent suicide attempt or psychiatric hospitalization within the last 6 months
- Active delirium
- Advanced dementia
- Currently participating in legacy programs through palliative care clinic
