The study includes ongoing monitoring of participants' experience and feedback, with a total participation period aligned to these timeframes.

Eligibility Criteria

Eligible

You may qualify if you...

  • Adults aged 18 years or older
  • Fluent in English
  • Able to provide informed consent
  • Able to complete questionnaires independently or with assistance
  • Diagnosed with serious illness defined as a health condition carrying high risk of mortality
Not Eligible

You will not qualify if you...

  • Untreated schizophrenia or bipolar disorder
  • Recent suicide attempt or psychiatric hospitalization within the last 6 months
  • Active delirium
  • Advanced dementia
  • Currently participating in legacy programs through palliative care clinic
