- Adult subjects 18 years or older
- English speaking
- Presenting to a participating pharmacy enrolled in the Community Pharmacy Enhanced Services Network - NY or associated with Tops Pharmacies
- Pharmacy must collaborate with Healthelink and referring partners and have capacity for a social needs screening program with Community Health Worker Training
- Pharmacy physical address must be located within Erie County
