Eligibility Criteria
You may qualify if you...
- Adult subjects 63 18 years
- English speaking
- Presenting to one of the participating pharmacies enrolled in Community Pharmacy Enhanced Services Network - NY or associated with Tops Pharmacies
- Pharmacies must be able to collaborate with Healthelink and referring partners and have staffing capacity for Social Determinant of Health screening with Community Health Worker training
- Pharmacies must be located within Erie County
