You may qualify if you...

  • Patients aged 18 years and older
  • Diagnosed with heart failure at the time of discharge
  • Left ventricular ejection fraction less than 50% based on imaging near discharge
Not Eligible

You will not qualify if you...

  • No means of communication with healthcare staff (phone, computer, messaging, etc.)
  • Do not reside in Vietnam after discharge
