Who Can Participate
Eligibility Criteria
You may qualify if you...
- Clinician who actively provides home-based primary care (HBPC) to patients enrolled in one of our partner home-based primary care programs at least part time.
- Patients receiving home-based primary care.
- Patients aged 60 years or older.
- Patients seen by a clinician participating in the study.
You will not qualify if you...
- HBPC clinicians who are not physicians, nurse practitioners, or physician assistants actively providing care at partner sites at least part time.
- Patients not aged 60 years or older.
- Patients not enrolled in a site-specific partner HBPC or not treated in their primary residence.
