They will also study how well the program fits within community settings, the factors supporting its success, and any barriers, with the goal of empowering Native American communities to improve nutrition, reduce health disparities, and strengthen local food systems over the long term.

Eligibility Criteria
You may qualify if you...
- Adults aged 55 years or older
- Live on the Flathead Reservation
- Self-identify as Native American
- Experience food insecurity as determined by a two-question screener
- Have a diagnosis of diabetes
- Are pre-diabetic based on specific blood test results or a risk screener
You will not qualify if you...
- Individuals with end-stage renal disease
- Those whose health provider advises against participation
