Using Male Peer Networks to Increase HIV Self-Testing and PrEP Use Among Men in Eastern Zimbabwe

Researchers are exploring new ways to engage men in Sub-Saharan Africa with HIV testing, treatment, and prevention services to help reduce HIV infections. Men often have lower participation in HIV prevention, increasing their risk and contributing to new infections in women. This study focuses on how distributing HIV self-testing kits through male peer networks, supported by phone assistance and better risk awareness, affects the use of HIV prevention medicine called PrEP among men in Eastern Zimbabwe. The study uses a cluster-randomized design with 44 community clusters, each with about 81 men, divided into two groups one receiving peer-distributed HIV self-testing kits and the other receiving kits from community health workers as usual care. In the peer distribution group, initial distributors get kits for themselves and peers, who can also become distributors, spreading the testing through social networks. Support is provided via a toll-free helpline and SMS risk assessments to encourage quick clinic visits for PrEP. Distributors and participants receive small payments for their involvement. Participants will be followed for six months to track PrEP initiation, clinic attendance, HIV testing, and treatment starts. The study team collects data through calls, referrals, and clinic records, including blood tests to confirm medication adherence. They will also evaluate how well the program is implemented and its effects on HIV prevention. The total involvement includes enrolling distributors, distributing kits, follow-up calls, clinic visits, and monthly contact for support and adherence checks.
