What this Trial Is About
Researchers are studying advanced soft tissue sarcoma in an open-label phase 12 trial that tests increasing doses of Lurbinectedin combined with fixed doses of Ipilimumab and Nivolumab given intravenously. The study aims to find the highest safely tolerated dose and evaluate safety and potential effectiveness in participants with this condition. The trial includes participants with locally advanced or metastatic soft tissue sarcoma, both previously treated and untreated depending on the study phase.
The study starts with a dose escalation phase using a cohort of three design, where participants receive escalating doses of Lurbinectedin every three weeks, alongside fixed doses of Ipilimumab every 12 weeks and Nivolumab every two weeks. Dose escalation proceeds after safety evaluation for six weeks to determine the maximum tolerated dose or maximum administered dose. In the phase 2 part, additional participants receive treatment at the established dose levels to assess safety and potential effects over time.
Participants will be monitored closely during treatment, which can continue up to one year or until disease progression or unacceptable side effects occur. Researchers will assess safety, response rates, progression-free survival, and overall survival over months to years. Evaluations include clinical and laboratory tests, with special attention to dose-limiting toxicities during the initial six weeks. The trial may include up to 18 doses of Lurbinectedin, 26 of Nivolumab, and 5 of Ipilimumab, with ongoing follow-up to collect data on treatment impact.

Factors
Investigational Combination (Study Treatment)
Standard First-Line Chemotherapy
What it is
An investigational drug regimen combining lurbinectedin with immune checkpoint inhibitors ipilimumab and nivolumab, explored in a single-arm dose escalation and expansion study for adults with advanced soft tissue sarcoma.
Conventional first-line chemotherapy based on doxorubicin, often given as monotherapy or sometimes in combination with ifosfamide, is the established standard treatment for newly diagnosed advanced or metastatic soft tissue sarcoma.
Role in the trial
This is the only treatment group in the trial, assessing the combination’s safety, tolerability, and early antitumor activity.
Standard therapy is not randomized in this trial but represents the externally accepted first-line benchmark treatment at the same care stage.
Primary use or intent
Designed to determine the maximum tolerated dose and early signs of benefit of the lurbinectedin, ipilimumab, and nivolumab combination as treatment for advanced soft tissue sarcoma.
Intended to control disease, improve symptoms, and prolong survival in patients with advanced, unresectable, or metastatic soft tissue sarcoma.
How it is given or performed
Participants receive intravenous lurbinectedin with fixed intravenous doses of ipilimumab and nivolumab on a repeating cycle, with dosing adjusted based on safety escalation during the study.
Standard chemotherapy is delivered intravenously, most commonly with doxorubicin every 3 weeks in cycles, sometimes with ifosfamide as a combination.
Evidence status / maturity
Ongoing early-phase clinical investigation; results on efficacy and safety in this combination and disease are not yet established.
Supported by established evidence from multiple clinical studies and guidelines as the primary initial therapy for advanced soft tissue sarcoma, though rates of durable response remain modest.
Important limitations
Results are pending; critical limitations include the single-arm design, lack of comparator, and unknown long-term activity or risk profile in soft tissue sarcoma.
Standard regimens are associated with considerable hematologic and cardiac toxicity, with overall response rates under 30% and most patients eventually progressing.
