Researchers are evaluating a new model of care called the GPS project aimed at improving medication use for seniors who are undergoing cognitive assessments or suffering from major neurocognitive disorder (MCND) at home. This model seeks to reduce the number of medications seniors take, limit inappropriate prescriptions, and decrease their treatment burden while maintaining cognitive health, quality of life, and independence. The study compares seniors receiving this new approach with those receiving usual care to assess its impact on pharmacotherapy and support for autonomy. The intervention involves nurses, pharmacists, and doctors working together in Family Medicine Groups (FMGs) and collaborating closely with home care teams. Pharmacists conduct detailed medication reviews and interviews with seniors and caregivers to reconcile medications and identify any medication-related problems. They then create personalized care plans and follow-up schedules in coordination with the healthcare team. The study includes around 200 seniors in the intervention group and another 200 in a control group receiving standard care. Participants will be followed for about six months after enrollment, with a total inclusion period of 12 months. Researchers will monitor changes in the number of prescribed medications at the start, three months, and six months after joining the study. They will also assess satisfaction among seniors, their families, and healthcare professionals involved. The study focuses on the safety and effectiveness of the new care model without testing any drugs or devices.
