Researchers are conducting a prospective, multicenter cohort study to investigate the relationship between lung and intestinal microbiomes, host susceptibility, and the prognosis of patients with severe pneumonia. The study addresses gaps in understanding due to previous small sample, single-center studies, focusing on how microbiome changes relate to disease progression and risk in different patient groups. This research aims to clarify the pathogenesis of severe pneumonia caused by various pathogens by collecting clinical samples and data from ICU patients between 2023 and 2025. The study involves collecting sputum, alveolar lavage fluid, feces, and whole blood samples from severe pneumonia patients admitted to intensive care units. Clinical metagenomics will be used to evaluate pathogens present in these samples. Patients with new lung infections requiring mechanical ventilation or high flow oxygen therapy meeting specific criteria will be included. The study will track changes in lung and intestinal microbiomes and host susceptibility to understand disease progression risks. Participants will undergo sample collection and clinical data gathering during their ICU stay, with the study monitoring outcomes over 60 days to assess the correlation between lung microbiome profiles and disease prognosis. Researchers will analyze these samples alongside patient clinical information to better understand severe pneumonia's development and outcomes. This comprehensive approach aims to provide valuable insights into microbiome-host interactions in severe pneumonia.
