Ending tuberculosis TB in Ethiopia by 2035 requires reducing TB notifications by at least 15% annually through effective prevention and care strategies. This trial evaluates whether a community-wide enhanced TB prevention therapy TPT combined with optimized TB care can accelerate the drop in TB cases. The study also aims to assess community acceptance, provider support, policymaker views, and cost-effectiveness to help develop a TB elimination framework for high-burden areas in Ethiopia. Participants will be in one of two groups one receiving an enhanced community-wide TB intervention, including intensive screening, health education, contact investigations, targeted TB diagnosis, optimized HIV screening and TPT for people living with HIV, and community awareness activities the other group will receive standard TB care per Ethiopias national guidelines. The study lasts up to two years, with ongoing semi-annual TB screenings. Throughout the study, participants will undergo regular TB screenings every six months to measure TB prevalence at the population level. Researchers will also track changes in TB treatment outcomes from the start to two years later. The study involves community-based monitoring and health education efforts, with a total participation period of up to two years to evaluate the impact of the enhanced interventions on TB rates and treatment success.
