Researchers are investigating liver transplantation outcomes in adults with cirrhosis and severe acute-on-chronic liver failure (ACLF). The study compares one-year survival rates after liver transplantation among patients with ACLF-2 or ACLF-3 at the time of transplant, those with decompensated cirrhosis without severe ACLF, and patients with ACLF-2 or 3 who are not listed for transplantation. This research also aims to understand factors influencing transplant listing decisions, waiting list outcomes, and post-transplant survival and quality of life. Participants are divided into three groups: those listed for liver transplant with ACLF-2 or 3, those listed with decompensated cirrhosis but without severe ACLF, and those with ACLF-2 or 3 assessed but not listed for transplant. The study follows these groups for one year after liver transplantation or after inclusion for those not transplanted. The observational protocol monitors these patients to gather detailed information on transplant success and survival. During the study, researchers will track patient survival for one year post-transplant or post-inclusion for non-transplanted patients. They will assess reasons for transplant listing decisions, outcomes on the waiting list, graft acceptance, and predictors of mortality or delisting. Quality of life after transplantation will also be compared between groups. Participants undergo clinical evaluations and are monitored closely throughout the study to better understand liver transplant outcomes in severe liver disease.
