This research aims to compare 1-year survival rates of patients undergoing liver transplantation LT with different severities of liver failure, including those with acute-on-chronic liver failure ACLF grade 2 or 3 and those with decompensated cirrhosis without severe ACLF. It also assesses survival for patients with severe ACLF who are not listed for transplantation. The study explores factors influencing transplant decisions, outcomes on waiting lists, and post-transplant survival and quality of life. Participants are grouped into three categories those listed for LT with ACLF-2 or 3 at listing or who develop these conditions while waiting those listed with decompensated cirrhosis but without ACLF-2 or 3 and poor liver function MELD score above 20 and those with ACLF-2 or 3 assessed for listing but ultimately not listed. The study follows all groups for one year after transplantation or inclusion to observe outcomes. Participants will be monitored for survival, transplant listing decisions, waiting list outcomes, graft acceptance characteristics, and quality of life after LT. Data collection includes clinical evaluations and tracking of patient and graft survival for one year. The study is observational, collecting information to improve understanding of transplantation outcomes and organ allocation for patients with severe liver disease.
