Researchers are evaluating the effects of norepinephrine versus phenylephrine in maintaining blood pressure during spinal anesthesia for elective cesarean delivery. This randomized, double-blind, controlled trial aims to determine if norepinephrine provides neonatal outcomes, measured by umbilical artery pH, that are at least as good as those with phenylephrine. The study also compares the impact of these drugs on maternal blood pressure and heart rate during surgery. Participants will be randomly assigned to receive either a continuous infusion of norepinephrine or phenylephrine starting at the time of spinal anesthesia. The infusion rate will be manually adjusted every minute based on systolic blood pressure readings to keep values near baseline. The medications are prepared in standardized doses, and the spinal anesthesia includes hyperbaric bupivacaine and fentanyl. Blood pressure and heart rate will be closely monitored throughout the procedure, with protocols in place to manage episodes of hypotension, hypertension, or bradycardia. During the study, umbilical arterial blood will be collected immediately after delivery to analyze pH as the primary outcome. Researchers will also record maternal blood pressure and heart rate continuously, noting any adverse events. The trial will take place at Hassan II University Hospital in Morocco, with careful data collection and management to assess the safety and effects of the two vasopressors during cesarean delivery under spinal anesthesia.
