Borgholm

Researchers are evaluating the effects of structured health dialogues with individuals aged 67 to 84 years living in Borgholm municipality, Sweden. This study aims to monitor quality of life, morbidity, healthcare needs, and lifestyle factors over six years by comparing groups within Borgholm and a matched control group from neighboring municipalities. It also includes cost-effectiveness analysis and explores participants' motivations and experiences with health dialogues. Additionally, machine learning models will be developed and validated to predict functional decline and healthcare needs using clinical data and digital motion analysis with Skeleton Avatar Technology. Participants in the intervention group receive a health dialogue conducted by a nurse practitioner. This includes a pre-visit assessment with a walking test, measurements, and blood tests, followed by a detailed 60-minute health dialogue covering living conditions, activity, and lifestyle. A personalized health improvement plan is created with referrals if needed. Three months after the dialogue, a follow-up call evaluates and adjusts the plan. The study includes intervention, randomized control, and matched control groups. Participants will complete postal or web-based questionnaires at baseline, 16 months, and six years to assess health-related quality of life using EQ-5D and ICECAP-O measures. Healthcare registry data will be collected to monitor health outcomes and service use. Qualitative interviews explore participants' perceptions and motivations. Researchers will analyze differences between groups, risk levels, and participation effects while monitoring cost-effectiveness and long-term health outcomes over the six-year period.
