  • Living in Borgholm municipality or in one of 7 matched Demographical Statistical Area in Kalmar county
  • Age between 67-84 years
Not Eligible

You will not qualify if you...

  • Living in a nursing home
  • Not speaking or understanding swedish language

Screening

Duration - 2 to 4 weeks

Participants are screened for eligibility to participate in the trial.

Health Dialogue Intervention

Duration - Initial session plus follow-up at 3 months

Participants attend a health dialogue session including physical tests and a structured conversation with a nurse practitioner to discuss health status and create a personalized health plan. A follow-up call is conducted at 3 months to evaluate and adjust the plan.

Long-term Monitoring

Duration - Up to 6 years

Participants complete questionnaires and their health outcomes are monitored via medical and social care records over several years to assess quality of life, health service use, and other health indicators.
