This research aims to evaluate the self-care abilities of farmers and fishermen diagnosed with cardiometabolic diseases such as hypertension, heart disease, hyperlipidemia, and type 2 diabetes. The study focuses on this population along Taiwan's southwestern coast, where these diseases are prevalent and medical resources are limited. It integrates Dorothea Orem's nursing self-care theory with the American Heart Association's Life Essential 8 concept to develop tailored educational materials and tools addressing both health and socioeconomic challenges faced by this group. The study includes developing self-care teaching materials and a Monopoly board game designed for educational and entertaining use, reflecting the cultural and economic background of the participants. The first year involves assessing self-care ability and biochemical indicators and introducing the CMDs self-care manual. From the sixth month, self-care progress is evaluated, and from the seventh to twelfth month, participants receive telephone follow-ups every two months. The intervention group additionally engages in multiple board game sessions led by trained community seed teachers to enhance health education. Participants will be regularly assessed for changes in self-care ability, cardiometabolic status, and inflammatory markers through various measurements and statistical analyses over 12 months. The study measures how well participants can reduce cardiometabolic risks by improving self-care. The project also aims to create reliable and engaging educational tools for use by health bureaus and primary healthcare units, with the potential for patents and technology transfer to support sustainable health improvements in this community.
