Researchers will measure the reduction in cardiometabolic risks through improved self-care ability over 12 months. The study aims to produce effective, culturally appropriate educational tools to support health bureaus and primary healthcare units in this community.

Brief Title

To Understand the Self-care Ability of Farmers and Fishermen With CMDs (Hypertension, Heart Disease, Hyperlipidemia, Type 2 Diabetes), Whether They Are Aware of CMDs, Their Treatment Status, Treatment Effectiveness, and Their Ability to Describe Known and Practice the Self-care Content of CMDs.

You may qualify if you...

  • Farmers and fishermen diagnosed with cardiometabolic diseases
  • Aged between 18 and 75 years old
  • Able to care for their daily life and walk to the community activity center
  • Can communicate in Taiwanese or Mandarin
  • Agree to participate in the second and third years of the study
Not Eligible

You will not qualify if you...

  • No household registration or living in an unstable place in the study area
  • History of stroke or myocardial infarction
  • Kidney or heart failure with limited water intake
  • Cognitive or mental disorders

Assessment and Education

Duration - 1 to 5 months

Participants complete assessments of self-care ability and biochemical indicators. They receive the CMDs self-care teaching material manual and have their referral treatment status evaluated. Questions are answered during this time.

Monopoly Board Game Intervention

Duration - During months 1 to 5

Participants in the experimental group play the Monopoly board game 8 times in small community groups, with health education Q&A sessions before each game activity led by a group leader.

Self-care Process Assessment

Duration - At month 6

A self-care process assessment is conducted in each community to evaluate participant progress.

Follow-up Telephone Visits

Duration - Months 7 to 12

Participants receive telephone visits every 2 months to support and monitor their self-care. Each call lasts 5 to 10 minutes.
