What this Trial Is About

This research aims to compare the pain relief and safety of two nerve block methods in adults undergoing elective shoulder stabilization surgeries, specifically Bankart repair or Latarjet procedures. The study focuses on evaluating whether the Modified Pericapsular Nerve Group PENG Block combined with Suprascapular Nerve Block SSNB can provide similar or improved postoperative pain control compared to the standard Interscalene Brachial Plexus Block ISB. The trial involves 70 adult patients classified as ASA I or II and seeks to reduce complications associated with the current standard technique. Participants will be randomly assigned to receive either an ultrasound-guided Modified PENG Block combined with SSNB or an ultrasound-guided ISB before they undergo general anesthesia for their shoulder surgery. The Modified PENG Block uses 20 mL of 0.25% bupivacaine, and the SSNB uses 10 mL of 0.25% bupivacaine, while the ISB involves 20 mL of 0.25% bupivacaine alone. These nerve blocks are performed prior to surgery to manage postoperative pain. The study is conducted at a single center and follows a parallel, double-blind design. During the study, participants pain intensity will be measured using the Visual Analog Scale VAS at 24 hours after surgery. Additional assessments include opioid consumption, time to first rescue analgesia, functional recovery, range of motion, and monitoring for block-related side effects such as respiratory compromise, motor weakness, Horner syndrome, hoarseness, and local anesthetic toxicity. Follow-up evaluations will continue according to the study protocol, ensuring comprehensive safety and efficacy monitoring throughout the postoperative period.
