What this Trial Is About
Researchers are evaluating the feasibility of combining radiation therapy with oral vancomycin before administering CAR T-cell therapy for patients with large B-cell lymphomas. This pilot trial aims to study whether this combination approach can be used safely and effectively in patients undergoing standard of care CD19 CAR T-cell treatment. The trial is sponsored by Abramson Cancer Center at Penn Medicine and is focused on treatment outcomes for these lymphoma patients. Participants will receive oral vancomycin 125 mg four times daily before undergoing standard radiation therapy targeted to at least 50% of their lymphoma sites. Following radiation, participants will receive the standard of care anti-CD19 CAR T-cell therapy. This single-group study involves a sequence of treatment steps including vancomycin intake, radiation delivery, and CAR T-cell infusion, designed to assess safety and response. During the study, participants will be monitored for side effects such as cytokine release syndrome and adverse events from the start of vancomycin treatment through 28 days after CAR T-cell infusion. Researchers will also evaluate infection risks, including vancomycin-resistant enterococcus, and measure lymphoma response for up to 12 months after CAR T-cell therapy. Participants will be assessed with scans and clinical evaluations throughout the treatment and follow-up periods to track safety and effectiveness.
