Eligibility Criteria

Eligible

You may qualify if you...

  • Community-dwelling adults aged 65 years or older
  • Reported at least two falls in the six months before the intake visit
  • Able to drive independently at the time of intake
  • Able to walk at least 10 feet with or without an assistive device (excluding wheelchair users)
  • Able to make medical decisions independently and sign consent forms
  • Reside in a community setting, not in a hospital or long-term care facility
  • Speak, read, and understand English fluently
