What this Trial Is About

Gastric cancer and gastroesophageal junction adenocarcinoma are serious conditions often diagnosed at an advanced stage, leading to severe complications and reduced quality of life. Current standard treatments involve chemotherapy combined with targeted agents or immunotherapy, but survival rates remain low. This study is evaluating trans-arterial infusion chemotherapy TAIC as a promising localized treatment option for patients with locally advanced, unresectable tumors who are not candidates for surgery or standard chemotherapy. Participants in this study will receive TAIC using a FOLFOX-based chemotherapy regimen delivered directly into the arteries feeding the tumor at weeks 0 and 4, combined with intravenous FOLFOX chemotherapy at weeks 2 and 6. The TAIC involves delivering oxaliplatin, leucovorin, and fluorouracil via transarterial infusion. Patients who test positive for the HER2 genetic mutation may also receive trastuzumab alongside their chemotherapy. This approach aims to maximize drug concentration in the tumor while minimizing systemic side effects. Throughout the 10-week treatment period, participants will be monitored closely with quality of life assessments before each treatment and after completion. Tumor response will be evaluated by investigators at weeks 4 and 8 using established criteria. Safety, disease control rate, and objective response rate will also be tracked. This study offers detailed monitoring of treatment effects and side effects, with the goal of improving outcomes for patients with advanced gastric cancer or gastroesophageal junction adenocarcinoma.
