You may qualify if you...

  • New patients having their first office call or telemedicine consultation with an AIMS provider
  • Established patients of the AIMS Institute
  • Adults over 18 years old who can understand the study and provide signed informed consent
  • Children under 18 with a parent or guardian able to understand the study and provide signed consent
Not Eligible

You will not qualify if you...

  • Cannot read or understand English well enough to sign consent and complete questionnaires
  • Unwilling to participate in the AIMS Institute observational study
