Eligibility Criteria
You may qualify if you...
- Patients who are new to the AIMS Institute for a first office call or telemedicine consultation
- Established patients of the AIMS Institute
- Adults 18 years or older able to understand and provide signed informed consent
- Children under 18 years with a parent or guardian able to understand and provide signed informed consent
You will not qualify if you...
- Unable to read or understand English sufficiently to complete consent forms and questionnaires
- Unwilling to participate in the AIMS Institute observational study
