  • At least one visit to a stem cell transplant center beginning January 1, 2021
  • Proposed to undergo or have undergone hematopoietic stem cell transplantation
Not Eligible

You will not qualify if you...

  • Long-term follow-up information not available due to any reason such as serious concomitant disease
  • Alcohol or drug addiction affecting ability to follow study requirements
  • Conditions that could jeopardize patient safety or adherence to the protocol according to investigator's opinion
