- At least one visit to a stem cell transplant center beginning January 1, 2021
- Proposed to undergo or have undergone hematopoietic stem cell transplantation
You will not qualify if you...
- Long-term follow-up information not available due to any reason such as serious concomitant disease
- Alcohol and drug addiction affecting ability to follow study requirements
- Conditions that could jeopardize patient safety or affect adherence to the protocol according to investigator's opinion
