- Proposed to undergo or have undergone hematopoietic stem cell transplantation

- Long-term follow-up information not available due to any reason such as serious concomitant disease
- Alcohol or drug addiction affecting ability to follow study requirements
- Conditions that could jeopardize patient safety or adherence to the protocol according to investigator's opinion
