Eligible

You may qualify if you...

  • Aged 21 years and above
  • Diagnosed with type 2 diabetes mellitus
  • Most recent HbA1c measurement of 6.5% or higher
  • Physically able to exercise
  • Able to read, write, and converse in English
  • Able to comply with study procedures
Not Eligible

You will not qualify if you...

  • Requires assistance with basic activities of daily living (BADL)
  • Planned major operation or surgical procedure within 6 months from recruitment
  • Currently pregnant or lactating

Outpatient Treatment
