This trial aims to assess its effectiveness, adoption, and sustainability in the less-controlled community settings, preparing for its large implementation in the future.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Aged 21 years and above
- Diagnosed with type 2 diabetes mellitus
- Most recent HbA1c is 6.5% or higher
- Physically able to exercise
- Able to read, write, and converse in English
- Able to comply with study procedures
You will not qualify if you...
- Require assistance with basic activities of daily living
- Have a planned major operation or surgery within 6 months of recruitment
- Currently pregnant or lactating
