Researchers are evaluating a mobile health app called EMPOWER combined with community-based health coaching to support adults with type 2 diabetes. The study aims to see how well these personalized interventions help patients manage their condition outside of traditional clinical settings. EMPOWER includes features like logging, gamification, educational resources, and personalized nudges to encourage healthy behaviors. This trial focuses on the apps effectiveness, adoption, and sustainability in real-world community environments, preparing for wider future use. Participants will use the EMPOWER app along with a Fitbit wearable device to track physical activity and health data. They will also receive community-based health coaching and may take part in other structured programs designed to improve diabetes management. The intervention is behavioral and continuous, helping patients set goals, develop action plans, and maintain motivation through regular support. During the study, participants will be monitored for 6 months with assessments including blood sugar control HbA1c, physical activity levels, medication adherence, quality of life, patient activation, and healthcare costs. Researchers will also track maintenance of effects on blood pressure and cholesterol levels. Participants need to be able to exercise, communicate in English, and follow study procedures. The trial involves ongoing data collection through the app and wearable device, with careful monitoring of health outcomes and participant engagement.
