Researchers are evaluating a community-based outreach program designed to improve screening, diagnosis, and treatment for breast, lung, and prostate cancers among underserved adults. The study uses the Reach, Effectiveness, Adoption, Implementation, Maintenance (RE-AIM) framework to understand how well the program works and its impact on cancer care in the community. The program involves identifying adults who meet cancer screening criteria and referring those with positive screening results for further diagnosis. Additionally, community stakeholders and healthcare providers are engaged through focus groups to provide feedback on the program's operations and help improve cancer screening and treatment pathways. Participants will be involved in the study from enrollment through a 3-month follow-up period, totaling 12 months. Researchers will assess aspects of the RE-AIM framework, patient health-related quality of life using the PROMIS Global Health scale, and patient satisfaction with navigation logistics. The study aims to identify barriers and facilitators to cancer care and to understand perceptions of patient navigators throughout the cancer care process.
