What this Trial Is About

Researchers are investigating how often acute exacerbations of chronic obstructive pulmonary disease (AE-COPD) trigger a heart condition called Takotsubo-stunning in patients admitted to the hospital. This observational study aims to understand the relationship between AE-COPD and this form of myocardial stunning, which may lead to acute heart failure and affect patient outcomes. The study is conducted at Sahlgrenska University Hospital and focuses on adults hospitalized for AE-COPD. Participants admitted for AE-COPD will be screened using echocardiography to detect signs of heart muscle dysfunction or regional wall motion abnormalities. If signs are found, patients will undergo detailed heart examinations shortly after admission and again 30 days later to see if the heart dysfunction improves, indicating Takotsubo-stunning. Additional data collected will include blood tests, ECG, chest X-rays, and questionnaires about symptoms. Patients who show signs of Takotsubo-stunning will be offered participation in another related study. During the hospital stay, vital signs, complications, and outcomes such as acute heart failure signs and major cardiac events will be monitored. Follow-up data on lung function and exercise capacity will be gathered after hospital care. The study measures include heart function assessment via echocardiography and clinical signs of heart failure during the first days of hospitalization. The total observation includes in-hospital events and a one-year follow-up for mortality and complications.

Your Study Journey

Screening

Duration - Up to 72 hours after hospital admission

Participants are screened for eligibility to participate in the trial.

1 screening and enrollment visit (in-person)

Diagnostic Evaluation

Duration - 30 days

Participants undergo echocardiographic screening to identify signs of regional wall motion abnormality and/or left ventricular dysfunction to detect Takotsubo-stunning. Screening may be repeated after 24 hours if initial screening is negative. If positive, full echocardiographic examinations are performed at baseline, 24 hours, and 30 days.

1 to 3 echocardiographic visits within 30 days

Monitoring

Duration - Average 5 days (duration of hospitalization)

Participants are monitored during hospital stay with blood sampling, ECG, chest x-ray, and registration of vital parameters and clinical signs. In-hospital complications and outcomes are recorded.

Daily assessments during hospital stay

Long-term Monitoring

Duration - Up to 1 year

Participants have follow-up assessments after hospital discharge, including re-evaluation of COPD through clinical routine tests such as spirometry, diffusion capacity, lung volumes, 6-minute walk test, and COPD assessment test.

Follow-up visits as per clinical routine
