- Patients admitted with acute decompensated heart failure
- Previous admission with decompensated heart failure in the 3 months before the current admission or
- Two or more admissions in the 6 months before the current admission

- Patients who are already receiving telemedicine management for heart failure

Duration - 6 months
Participants are helped to use telemedicine tools and receive telemedicine support from the hospital cardiology team as needed. Data on vital measurements, symptoms, and follow-up care are collected retrospectively at baseline, 3 months, and 6 months after admission.
