You may qualify if you...

  • Patients admitted with acute decompensated heart failure
  • Previous admission with decompensated heart failure in the 3 months before the current admission or
  • Two or more admissions in the 6 months before the current admission
Not Eligible

You will not qualify if you...

  • Patients who have refused to share their data for health-quality projects
  • Patients who are already receiving telemedicine management for heart failure

Participants are screened for eligibility to participate in the trial.

1 visit (in-person) during hospital admission

Monitoring

Duration - 6 months

Participants are helped to use telemedicine tools and receive telemedicine support from the hospital cardiology team as needed. Data on vital measurements, symptoms, and follow-up care are collected retrospectively at baseline, 3 months, and 6 months after admission.
