Researchers will monitor outcomes such as ulcer healing, amputation rates, mortality, and healthcare use. The study period includes up to 20 weeks of program participation with ongoing evaluation of program components and participant acceptability.

Eligibility Criteria
You may qualify if you...
- Adults 18 years or older with diabetes
- Hospitalized patients who have undergone amputation of 2 or more toes related to diabetic foot ulcers
- English-speaking patients able to communicate with available navigators
You will not qualify if you...
- Unable to understand the nature and scope of the study
- Currently enrolled in another clinical trial
- Planned discharge to acute or long-term care facility
- Planning to receive diabetic foot ulcer care outside the study area

Participants are screened for eligibility to participate in the trial.

Duration - Up to 20 weeks
Participants are assigned a dedicated diabetic foot ulcer patient navigator who assists with care coordination and support to improve wound healing.
Weekly visits or phone encounters with the patient navigator, plus additional contacts as needed
Duration - Up to 20 weeks after discharge
Participants are monitored for wound healing and other health outcomes after hospital discharge.
