The project uses advanced imaging methods such as structural MRI and PET scans to monitor brain changes and biomarkers at different stages of Alzheimer's progression. Participants will undergo regular assessments including neuropsychological tests, brain imaging, and biomarker analysis to track disease development over time. The study measures the prevalence of gene mutations and patterns of genetic, biofluid, imaging, and cognitive markers over multiple years. It also evaluates changes in brain structure, metabolism, amyloid and tau deposition, and tests non-drug interventions. The total participation duration varies, with some measures assessed yearly for up to 10 years, supporting long-term monitoring and data collection.

Eligible

You may qualify if you...

  • Written informed consent obtained from the participant or a legal guardian before any study procedures
  • At least two first-degree relatives in a family have Alzheimer's disease, with patients in at least two generations
  • At least one family member with normal cognitive function older than the family's average age of onset
  • Pedigrees carrying FAD pathogenic genes (APP, PSEN1, PSEN2)
  • Participants are older than 18 years
  • Participants may be cognitively normal or have dementia but not bedridden
  • Ability to provide two reliable informants who can supply clinical information
  • Dementia diagnosed using DSM-IV-R criteria
  • Alzheimer's diagnosis based on NINCDS-ADRDA or NIA-AA criteria
  • Mild cognitive impairment diagnosed according to Petersen criteria and Lopez classification

You will not qualify if you...

  • Dementia caused by other conditions such as depression, psychiatric illnesses, thyroid dysfunction, encephalitis, multiple sclerosis, brain trauma, brain tumor, syphilis, AIDS, Creutzfeldt-Jakob disease, vascular dementia, frontotemporal dementia, dementia with Lewy bodies, or Parkinson's dementia
  • MRI and lab tests that do not support an Alzheimer's diagnosis
  • Severe circulatory, respiratory, urinary, digestive, hematopoietic diseases, or cancer
  • Severe psychiatric illness or severe dementia interfering with assessments
  • History of alcoholism or drug abuse
  • Pregnant or lactating women
  • No reliable informants available
  • Lumbar puncture contraindications such as coagulation disorders, recent lumbar surgery, or repeated cerebrospinal fluid leakage
  • MRI contraindications including electronic or magnetic metal implants, metallic eye foreign bodies, or brain aneurysm clips
  • For normal controls: abnormal cognitive test scores, history of brain injury or infarction, neurological or systemic diseases causing cognitive impairment, mental retardation, refusal of blood draw, MRI contraindications, or refusal to consent
