Chiropractic care includes posture correction, soft tissue release, spinal manipulation, education, strengthening, and ergonomic advice. Tai Chi classes will be held weekly for 16 weeks. During the study, participants will be assessed at baseline, 16 weeks, and 24 weeks for pain intensity, pain on movement, disability, quality of life, self-efficacy, postural awareness, pain catastrophizing, fear of movement, and interoceptive awareness. Researchers will also monitor recruitment, retention, and adherence rates throughout the average 18-month study duration. Participants will be followed for 8 weeks after interventions end to evaluate longer-term outcomes and safety.

Eligible

You may qualify if you...

  • Chronic nonspecific neck pain at least 5 days a week for at least 3 consecutive months
  • Neck pain averaged over past week of 3 or more on a 0 to 10 numerical rating scale
  • Neck Disability Index score of 5 or greater
  • Willing to participate in all study procedures and be randomized to any group
  • Fluent in English
Not Eligible

You will not qualify if you...

  • Received chiropractic care currently or in the past 12 months
  • Regular practice of Yoga, Tai Chi, or Qigong weekly in the past 6 months
  • Major systemic illness, unstable medical or psychiatric condition affecting study compliance
  • Any disability preventing exercise practice
  • History of stroke, carotid or vertebral artery dissection
  • Signs of myelopathy, carotid bruits, or pathological hypermobility
  • Neck pain caused by congenital spine deformity, neoplasm, inflammatory or neurological disorders
  • Neck pain with unstable radicular symptoms or radiculopathy
  • Pregnancy
  • Spinal injections within past 4 weeks
  • Neck pain involved in litigation
  • Cervical or spinal surgery within past year
  • Involved in a disability or accident claim

Treatment

Duration - 16 weeks

Participants receive their assigned intervention which may include up to 10 chiropractic sessions over 16 weeks, weekly Tai Chi classes for 16 weeks, and/or enhanced usual care with educational materials. Participants follow their assigned treatment plan for 16 weeks.
