Researchers are evaluating a program called Heart to Heart (H2H) designed to help patients with cardiovascular disease (CVD) make more informed decisions about cardiac rehabilitation (CR). The study focuses on patients hospitalized with CVD events who have low socioeconomic status and are eligible for CR. Participants will engage in the H2H intervention, which involves 4 to 8 coaching sessions delivered by telephone or video chat over a 3-month period. The Community Health Worker (CHW) leading these sessions will provide tailored education about cardiac rehab and cardiovascular disease, support to engage social networks, and assistance navigating the health system. During the study, participants will complete surveys at baseline, 3 months, and 6 months. Medical records will also be reviewed to track progress. The research team will measure how feasible and acceptable the program is, assess changes in barriers to cardiac rehab, patient activation, and count the number of cardiac rehab sessions attended. The total participation time for each person is up to 6 months from the start.

Eligibility Criteria
You may qualify if you...
- Patients hospitalized with cardiac events and eligible for cardiac rehabilitation at Johns Hopkins Hospital and Bayview Medical Center
- Low socioeconomic status defined by Medicaid insurance or high area deprivation index in Baltimore city
- Age 21 years or older
- Willing to be audio-recorded
You will not qualify if you...
- Formal diagnosis of dementia or cognitive impairment that would prevent study participation
- Formal diagnosis of severe psychiatric disorders (e.g., schizophrenia, bipolar disorder, major depression) that would prevent study participation
- Systemic illness limiting life expectancy (e.g., advanced cancer)
- Contraindications to cardiac rehabilitation as assessed by healthcare team
- Unwilling or unable to follow study procedures
- Unable to consent
- Unable to speak or understand English; all assessments conducted in English
