Eligibility Criteria
You may qualify if you...
- Age 70 years or older
- Having one or more chronic diseases
- Currently followed by a general practitioner due to chronic disease
You will not qualify if you...
- Diagnosis of dementia
- Known eye diseases or currently followed by a private ophthalmologist
- Unable to understand Danish
