Eligible

You may qualify if you...

  • Age 70 years or older
  • Having one or more chronic diseases
  • Currently followed by a general practitioner due to chronic disease
Not Eligible

You will not qualify if you...

  • Diagnosis of dementia
  • Known eye diseases or currently followed by a private ophthalmologist
  • Unable to understand Danish
