Participants and their caregivers will be assessed at the start, 4 weeks, and 12 weeks after starting the intervention. Data collected will include functional capacity, cardiac function, physical activity levels, psychological measures like depression and anxiety, quality of life, and mutuality between patient and caregiver. The study also tracks hospital admissions and care burden. Safety and progress will be monitored through regular evaluations, using statistical models to analyze changes over time. The total study duration from enrollment to final assessment spans 12 weeks.

Eligibility Criteria

Eligible

You may qualify if you...

  • CHF individuals aged 18 to 75 years
  • Diagnosed with chronic heart failure, any HF phenotype
  • NYHA class I, II, or III
  • 6-minute walk distance over 150 meters
  • Stable disease phase for at least 4 weeks
  • Able to practice Tai Chi independently without musculoskeletal issues
  • Sedentary lifestyle sitting more than 8 hours per day in past month
  • Own a smartphone
  • Willing to participate and provide informed consent
  • Caregivers aged 18 or older
  • Primary home caregiver with emotional or social ties to CHF individual
  • Able to perform Tai Chi without assistance
  • Willing to participate and provide informed consent
Not Eligible

You will not qualify if you...

  • Planned surgery within next 6 months
  • Severe comorbidities such as cancer, liver or kidney failure, or psychiatric disorders
  • Plans to enter nursing home or care facility
  • Regular Tai Chi practice or other exercise at least 3 times per week for 20 minutes each in past 3 months
  • Currently participating in similar or related studies
  • Caregivers who are formal care providers such as nurses

Treatment

Duration - 12 weeks

Participants in the HDTC group attend a 30-minute educational seminar, a 1-hour dyadic coaching session, a 1-hour preliminary Tai Chi training session, followed by 12 weeks of Tai Chi training with gradually increasing frequency and duration. Participants in the usual care group receive a 30-minute online education and monthly nurse-led telephone support.
