What this Trial Is About
Researchers are evaluating the clinical efficacy and safety of a new pain management approach combining transversus abdominis plane block TAPB with patient-controlled intravenous analgesia PCIA to improve postoperative gastrointestinal recovery in patients undergoing major abdominal surgery. This prospective, randomized, double-blind trial involves 132 adult patients aged 18 to 80 scheduled for elective small bowel or colorectal surgery. The study aims to test whether this multimodal analgesia can reduce postoperative ileus and speed up recovery of gastrointestinal function. Participants are randomly assigned to one of three groups receiving different postoperative pain control regimens TAPB with 0.375% Ropivacaine plus Sufentanil PCIA TAPB with 266 mg Liposomal Bupivacaine plus Sufentanil PCIA or TAPB with 266 mg Liposomal Bupivacaine plus Oxycodone PCIA. Each group receives the assigned local anesthetic block combined with intravenous patient-controlled analgesia to manage postoperative pain. This design allows comparison of the effects of the different analgesia combinations on recovery. During the study, participants will be monitored for gastrointestinal function recovery using the I-FEED scoring system over the first 7 postoperative days. Pain levels, opioid use, time to first ambulation, bowel movements, hospital stay length, recovery quality, and adverse events are also assessed through questionnaires and clinical observations. The study includes careful safety monitoring and aims to complete follow-up within two weeks after surgery.
