Participants will either receive this comprehensive caregiver support model or standard caregiver education and information leaflets. During the study, caregivers will complete three surveys: one at the start of the patient's hospital stay, another around discharge, and a final one one month after discharge. An optional interview may also be offered. Researchers will evaluate caregiver preparedness, resourcefulness, burden, anxiety, depression, and quality of life. Patient data on emergency visits and readmissions will also be reviewed. The study will continue to collect data up to two months after patient discharge to assess the lasting effects of these initiatives.

Eligibility Criteria

Eligible

You may qualify if you...

  • 21 years of age and above
  • Main or secondary caregiver of an older person receiving home-based care
  • Able to read and converse in English
  • Has a mobile smartphone and is able to navigate basic functionalities on mobile apps
Not Eligible

You will not qualify if you...

  • Participants who are cognitively impaired
  • Unwilling to download and utilise app
  • Unable to read or converse in English comfortably

Supportive Care

Duration - Duration of care recipients' hospital stay

Participants receive caregiver support services tailored to their needs during the care recipients' hospital stay, including access to resource libraries, training programs, telephonic support, and educational materials.

3 surveys: at beginning of hospital stay, around discharge, and 1 month after discharge; optional interview before or after discharge

Follow-up Evaluation

Duration - Up to 1 month after discharge

Participants complete follow-up surveys to assess outcomes such as caregiver preparedness, resourcefulness, burden, anxiety, depression, and quality of life up to 1 month after discharge. Some participants may also be contacted for interviews to discuss their experiences.
