Eligible

You may qualify if you...

  • Diagnosed with a neurocognitive disorder based on Alzheimer's disease, Parkinson's disease, Lewy body dementia, or mixed dementia according to DSM-5 criteria
  • Mini-Mental-Status-Examination (MMSE) score between 5 and 26 out of 30
Not Eligible

You will not qualify if you...

  • Mini-Mental-Status-Examination (MMSE) score less than 5
  • Unstable cardiovascular or other medical conditions that make participation unsafe
  • Planned transfer to another facility within the next 3 months
  • Acute behavioral problems
