What this Trial Is About

This research aims to evaluate the relationship between follicular volume at the time of final oocyte maturation and oocyte maturity in women with severely diminished ovarian reserve undergoing ovarian stimulation. Women with this condition and monofollicular growth face higher risks of premature ovulation and poor oocyte quality. The study explores whether measuring follicular volume, alongside follicular size, hormone levels, and past stimulation attempts, can better time the trigger for final oocyte maturation and potentially improve outcomes. Participants will receive ovarian stimulation using an antagonist protocol with Pergoveris gonadotropin at a starting dose of 450 IU. The antagonist begins on day 5 of stimulation. The trigger for final oocyte maturation is given when the leading follicle reaches 15 mm, following clinical standards for poor responders. On the day of the trigger and before oocyte retrieval, follicular volume will be measured using an automated ultrasound program during a routine ultrasound exam, without extending its duration. Oocyte pickup will follow standard clinic procedures for patients with fewer than seven follicles. Throughout the study, participants will undergo ultrasounds to measure follicular volume at key times, including before the trigger and prior to oocyte retrieval. Researchers will assess the correlation between follicular volume at trigger and oocyte maturity over one year, focusing on patients with fewer than four follicles measuring at least 11 mm. The study involves reproductive-age women seeking IVF treatment, with safety monitoring and adherence to standard clinical protocols. Total participation duration aligns with the ovarian stimulation and outcome measurement timeline.
