Participants will be randomly assigned to either a group receiving a geriatric assessment by a physician within 7 days after ICU discharge or a group without this early assessment. The assessment covers clinical, physical, psychological, and social factors including quality of life, depression, mobility, nutrition, cognition, and living conditions. Both groups will have a follow-up visit at 6 months with similar evaluations and additional data on hospitalizations, treatments, and family burden. During the study, participants will undergo assessments of medical history, ICU stay details, and comprehensive geriatric evaluations at discharge and six months later. Researchers will measure rates of completed assessments, frailty, hospital stays, treatment changes, quality of life changes, psychological and cognitive status, and organ function over time. This study is conducted by the University Hospital in Strasbourg and aims to improve care for elderly ICU survivors through better understanding of their needs after intensive care.

Eligibility Criteria
You may qualify if you...
- Patients over 75 years of age
- Admitted to the intensive care unit for any reason and survived the ICU stay
- Affiliated with a health insurance scheme
- Able to understand the study aims and risks and provide signed informed consent
- If confused at ICU discharge, have a close relative available to give signed consent with patient consent when possible
You will not qualify if you...
- Protected individuals under legal guardianship or similar protections
- Patients who are moribund or have a life expectancy less than 1 month as estimated by the clinician
- Patients not living in Bas-Rhin (required for 6-month visit)

Duration - Within 7 days after ICU discharge
Participants are either assessed by a geriatric physician in a comprehensive approach at the end of ICU stay or within 7 days after ICU discharge, or data are collected without a geriatric assessment.

Duration - 1 day at 6 months after ICU discharge
Participants attend a post-ICU consultation 6 months after ICU discharge where various assessments are performed including quality of life, cognitive and physical evaluations, and medical history updates.
