  • Showing signs of frailty as identified by the Gérontopôle Frailty Screening Test (GFST)
  • Willing and able to provide informed consent to participate
Not Eligible

You will not qualify if you...

  • Need for hospital admission
  • Estimated life expectancy less than one year

Screening

Duration - 2 to 4 weeks

Participants are screened for eligibility to participate in the trial.

Treatment

Duration - 10 weeks

Participants receive a comprehensive geriatric assessment followed by up to 10 weekly 1-hour group exercise sessions guided by a physiotherapist, along with non-pharmacological support including nutritional guidance, screening for swallowing and cognitive issues, sleep advice, social isolation support through community resources, and a medication review to optimize treatment.

Follow-up

Duration - Approximately 3 months

Participants continue to maintain physical activity and activation through integration with community resources such as gyms and civic centers to support ongoing healthy aging.
