What this Trial Is About
This phase III trial compares the effect of geriatric evaluation and management with survivorship health education (GEM-S) to usual care on patient-reported physical function in older survivors of cancer. Survivorship care for older adults of cancer usually consists of getting advice from their doctor. This advice may include how to do their daily activities, so they are less tired or how to manage multiple diseases, or long-term side effects from treatment. GEM-S may help improve the physical ability to perform activities of daily living, mental well-being, and memory in older survivors of cancer after chemotherapy. This study may help doctors learn if including GEM-S in their practices improves physical, mental and memory functions in their patients. The study may also help to understand how such care affects cancer patients and their caregivers' quality of life.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Participating clinical practices must identify at least one licensed oncologist or advanced practice practitioner willing to participate
- Oncologists/APPs must have no plans to leave or retire from the practice within one year of enrollment
- Practices must have support from oncology clinicians to participate in training and study procedures
- Cancer survivors must be 65 years or older
- Cancer survivors must have completed or will complete curative-intent treatment for solid tumor or lymphoma within the last 6 months
- Survivors receiving other treatments such as hormonal therapy, monoclonal antibodies, immunotherapy, or radiation are eligible
- Survivors must be willing and able to attend study visits or consent and assessments remotely
- Survivors must be willing and able to provide informed consent in person or remotely
- Survivors must speak and understand English and/or Spanish, with Spanish-speaking enrollment subject to resource availability and approval
- Caregivers must be 18 years or older
- Caregivers are selected by the survivor as someone helpful in health matters and need not live with the survivor
- Caregivers must speak and understand English and/or Spanish, with Spanish-speaking enrollment subject to resource availability and approval
You will not qualify if you...
- Cancer survivors with conditions preventing informed consent or participation (e.g., dementia) are excluded
- Caregivers unable to understand the consent form or study procedures due to cognitive, health, or sensory impairments are excluded
- Capacity to consent and participate will be determined by study coordinators and oncologists
