Assessments include physical and mental fitness, quality of life, lifestyle habits tracked by accelerometers and questionnaires, and care consumption analysis. Researchers monitor adherence, behavior persistence, and the economic impact of the APA program compared to usual care. Data collection and follow-up are performed by trained staff at Mon Stade.

Eligibility Criteria
You may qualify if you...
- Male or female
- 18 years of age or older
- Speaking and understanding French
- Affected by a chronic pathology according to a predefined list (long-term illness, obesity, high blood pressure, COPD, COVID-19 with persistent symptoms)
- Carrier of a written adapted physical activity prescription
- Able to attend Mon Stade twice a week for the 16-week adapted physical activity program
- Signed informed consent form
