Hepatocellular carcinoma HCC is a common primary liver cancer that often develops in people with chronic liver diseases like cirrhosis from hepatitis B or C. It causes significant mortality worldwide, with complications from liver disease and HCC ranking among leading causes of death. This research investigates novel treatments, focusing on radiofrequency ablation RFA and the role of NETosis, a biomarker linked to tumor growth and immune response, to improve outcomes after HCC treatment. The study compares two sedation approaches during RFA in adults with HCC tumors up to 3 cm. One group receives sedation with propofol and fentanyl combined with intravenous lidocaine bolus and infusion for 24 hours, while the other group receives propofol and fentanyl alone. The aim is to evaluate if lidocaine affects NETosis levels, recurrence rates, and overall survival after RFA. Blood samples will measure NETosis markers before and 24 hours after the procedure, and patients will be followed for one year. Participants undergo sedation and RFA with continuous monitoring of heart and respiratory function. Blood tests assess NETosis and inflammation markers at baseline and 24 hours post-procedure. Researchers will evaluate recurrence rates, survival, and biomarker changes over 12 months. The study includes randomized group assignment with blinded outcome assessment to determine if lidocaine reduces tumor recurrence by affecting NETosis in HCC patients treated with RFA.
