What this Trial Is About
The study will verify if a structured multidisciplinary approach (called iABC), aimed to improve the appropriate management of elderly AF patients with multimorbidity (the i-ABC group), would provide a clear evidence of an improvement in clinical conditions and quality of life compared to usual clinical care. The i-ABC group in AFFIRMO will follow the ABC pathway, focused on three domains: avoid stroke with anticoagulation (with optimized VKA or label-adherent DOAC use); better symptom management; and optimized management of associated cardiovascular and non cardiovascular comorbidities. The study will be conducted in Bulgaria, Denmark, Italy, Romania, Serbia and Spain .

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Outpatients aged 65 years or older
- Diagnosis of atrial fibrillation (paroxysmal, persistent, long-standing persistent or permanent) confirmed by ECG or Holter monitoring
- At least one additional long-term health condition such as hypertension treated with at least 2 drugs, coronary artery disease, peripheral artery disease, heart failure, stroke or transient ischemic attack, diabetes mellitus, COPD, or chronic kidney disease
You will not qualify if you...
- Mechanical prosthetic heart valve or moderate to severe mitral stenosis
- Patient unwilling to be enrolled or unable to provide written informed consent
- Patient unable to understand the study or attend follow-up visits
- Serious disease with life expectancy less than 12 months
- Participation in other interventional studies
- For iABC group sites, lack of a device suitable for using the iABC platform
