Participants will be postpartum mothers or parents aged 18 to 45 living in five target counties. The study will track outcomes such as the number of emergency department visits within the first 6 weeks postpartum and severe maternal morbidity rates up to one year postpartum. The research includes monitoring health outcomes, community trust, engagement in postpartum care, and the effectiveness of the home visits and communication strategies over the study duration.

- Identified as high risk pregnancy
- Postpartum mothers or parents aged 18 to 45 years
- Resident of one of the five target counties: Washington, Bolivar, Scott, Humphreys, or Carroll

- Not identified as having a high risk pregnancy
- Postpartum age under 18 years
- Cesarean birth for most recent delivery
- Non-resident of the five target counties

Duration - Up to 6 weeks postpartum
Participants receive 3 to 4 home visits with the PEN-3 Intervention Model, a sociocultural approach to support positive maternal health outcomes through referral and support processes.

Participants are monitored for maternal health outcomes including emergency department visits and severe maternal morbidity rates up to one year postpartum.
