Malignant Peripheral Nerve Sheath Tumors MPNSTs are rare, aggressive cancers that develop in soft tissues and are a leading cause of serious illness and death in people with neurofibromatosis type 1. This research investigates whether a new drug called BMS-986504, which targets a specific gene mutation called homozygous MTAP deletion, may work better than existing treatments for patients with unresectable MPNST. The study focuses on evaluating the safety and effectiveness of this treatment in people whose tumors cannot be surgically removed and who have this genetic mutation confirmed by next-generation sequencing. Participants will receive BMS-986504 orally at a dose of 600 milligrams daily on Days 1 to 28 of each 28-day cycle. Treatment will continue until the disease worsens, unacceptable side effects occur, or participants choose to stop. Those experiencing dose-limiting toxicity may have their dose reduced or be taken off the trial. The study enrolls participants in two stages nine in the first stage, and if at least one shows response, eight more in the second stage. This design helps assess safety and tumor response over time. Throughout the study, participants will undergo regular assessments including scans to measure tumor size and blood tests to monitor organ function and side effects. Researchers will track objective response rates, progression-free survival, overall survival, adverse events, and treatment discontinuations for up to two years. Participants will be monitored closely for safety and treatment effects during and after therapy, with follow-up evaluations continuing to capture outcomes and ensure well-being.

This table compares participation in this trial with other options available for this condition. The information is drawn from published research and the trial’s study details and is provided for educational purposes only.
| Factors | BMS-986504 Oral Therapy | Guideline-Recommended Systemic Therapy |
|---|---|---|
| What it is | Oral small-molecule investigational drug (BMS-986504) targeting patients with unresectable or metastatic malignant peripheral nerve sheath tumor (MPNST) and homozygous MTAP deficiency. | Standard systemic chemotherapy regimens, most commonly doxorubicin-based, as recommended by current clinical guidelines for unresectable or metastatic MPNST. |
| Role in trial or clinical care | Studied as a potential targeted treatment for people with MPNST lacking standard curative options. | Established systemic option provided as standard nonsurgical treatment, aiming to slow cancer progression and relieve symptoms. |
| Primary use in this context | Evaluated for potential tumor response, safety, and progression-free survival in unresectable or metastatic MPNST with MTAP deletion. | Used in routine clinical practice to manage disease progression and symptoms in advanced MPNST when surgical treatment is not possible. |
| How it is given or performed | Taken orally once daily in 28-day cycles at 600 mg (with possible dose reductions to 400 or 200 mg) until progression, unacceptable toxicity, or withdrawal, for up to 2 years. | Most commonly given as intravenous doxorubicin (sometimes in combination with ifosfamide or other agents), typically in repeating cycles, with close monitoring for side effects. |
| Evidence status | Efficacy and safety are under investigation; no published results yet available. | Systemic therapy for MPNST has shown limited objective response rates; chemotherapy is considered modestly effective and used when surgical cure is not feasible. |
| Important limitations | Novel investigational agent; limited trial size; only available in research setting. Full efficacy and safety profile not yet known. Only includes patients with MTAP deletion. | Chemotherapy, while established, often has limited efficacy and can cause substantial side effects; not all people achieve meaningful tumor response. |
