What this Trial Is About
Researchers are evaluating the use of patient-reported outcome measures PROMs to monitor symptoms and health-related quality of life HRQOL in individuals with advanced chronic kidney disease CKD. This study focuses on how routine symptom reporting using PROMs can improve symptom recognition, patient-provider communication, and HRQOL, addressing the current lack of standardized symptom monitoring in nephrology care. The trial is a pilot, randomized study involving 70 patients with advanced CKD. Participants are randomly assigned to one of two groups an intervention group using an electronic PROM tool IPOS-Renal to report symptoms monthly for 12 months, with symptom reports shared with their doctors and guidance provided for symptom management or a control group receiving usual care without structured symptom reporting. Both groups complete quality of life and symptom questionnaires at baseline, 6 months, and 12 months. The intervention supports clinician follow-up based on patient-reported symptoms. Throughout the study, participants complete surveys online, by mail, or by phone. Researchers collect clinical data at multiple time points up to 24 months and conduct interviews with patients and clinicians to understand experiences with symptom management and the intervention. The main outcomes include feasibility, acceptability, and preliminary effectiveness of the symptom reporting system, as well as clinical events like progression to kidney failure, hospital visits, and mortality. Participation lasts up to two years with ongoing data collection and evaluation.
